Cannot Test or distribute new form

i created new form , when go test form following error:
an error has occurred.
the form not exist. please re-enter web address link may misspelled
[ref: tegnvu9fniprdffimozumg, accesscontrolviolation]

 

i have tested on multiple computers, in ie , chrome, different adobe accounts.

How to Grab email address from webform and insert into form action

Thread: "Broken" background image with Super Key
