Is this Captcha code corrupt?

i have site has multiple newsletter signup forms on different pages. working correctly. today client emailed saying captcha wasn't working on 1 of pages. browsed page , found captcha image broken link symbol. checked html , found modified form text shown below. if down <div class="captcha"> appears "{module_captchav2}</div>" missing. see div class below "captchaimg" did not add , 3 "input id's did not include. cdata text different on form. made changed text green in html below highlight it.

 

can explain happened?

it not corrupt.

the situaiton viewed html of form on page, copied html , pasted there. may or may not have changed form action id match form (you should check) instead of needed captcha module, because copied html copied rendered captcha module. of course has session image have expired.
