this bit of broad question, let me try understand needs little better.

 

are looking use formscentral either create form or create form , collect , manage responses?  looking web/html form or pdf?

 

formscentral allows create web/html forms when submitted data sent formscentral , in paid level subscription can configure email notifications email results you, available in formscentral view , analyze.  web/html form can add link form on website, or web/html form can "embedded" web site (there embed code on distribute tab).

 

formscentral can create pdf form, pdf can either send responses formscentral web form same options (notifications etc), , can pdf form without "submit" button can add submit button of own email responses you.  requires having adobe acrobat edit form , add own submit button.

 

if use pdf see faq on pdf in web - not recommend "embedding" pdf web page, not have pdf open in browser, users should download pdf , fill out using free adobe reader: http://forums.adobe.com/docs/doc-4142
