Checkbox populates field with applicant's name.

depending on yes or no answer, i  populate field appliant name , info. (the applicant's name , info. entered in begining of application.)

 

what i'm looking is:

 

previously poupulated seperate fields in application:

name:  joe shmoe (applicants name)

phone:  555-555-5555

social security number: ###-##-####

 

a couple pages later, there question: working?

if yes then sectiion employment populate:

joe shmoe

555-555-5555

###-##-####

 

if no section left blank.

 

is possible?  

 

the application lengthy , has other instances type of pre-populate field great help. (are insured?,  own property?, ect.) if given answer section above, point me in productive direction.
